Preventive Dental Services include:

Screening and Dental Hygiene Diagnosis

Dental Prophylaxis (cleaning)

Digital Imaging (X-Rays, Intra-oral photos, etc.)

Fluoride Application and/or Remineralizing Agents

**Silver Diamine Fluoride and/or other Antimicrobial Desensitizing Agents

Preventive Dental Sealants (containing fluoride)

Individual Oral Hygiene Instruction

Temporary Filling Placement (fluoride-releasing)

Summary of Services Rendered and Referrals to
